I had a moment to review the situation and the terms and conditions of the casino as well as the free chips themselves state that only one free chip is valid between cash deposit/play sessions. In this case the win in question was the result of the 29th consecutive free chip without history of deposit and is not eligible for the $100 maximum cashout. The account verification deposit referred to was refunded in full at the time of denial of the requested withdrawal.
