Well, I DO know this casino has a terrible reputation, however, I have jumped through hoops and done all the things they asked me to do - and my account HAS been verified etc...
I won ALOT of $$ with that "no deposit free" bonus they give - and I mean alot (over $20,000 USD) but as per their terms, can only withdraw $100 USD from a win with a no deposit bonus. This being said, I have been waiting. and waiting some more... and more...
It is going on a month... maybe longer, this casino is so infuriating to deal with that I am finally now asking for help. I have given them my bitcoin address, and still - it's wait wait wait. it's only $100 USD, and it's the PRINCIPAL of it, I want them to honour their own TERMS!
I have attached some screenshots of my history (showing the dates of the withdrawals I have attempted) and the communication of having a verified account.
I find it interesting that they can legally take people's money - THAT is scary!

I have not withdrawn YET from this casino - this win is from a NO DEPOSIT free chip that you get when you join a casino.
So that also means I have never deposited here either.
I attached all the documents that show all the answers to your questions in my original request... And yes, I have made successful withdrawals (in general - at other casinos) MANY MANY times haha.. I am not new to online gambling.
I have provided this casino with all they require to process the winnings... and they are wasting time.
