Frankly, there is no tool to do that all in one step. I suspect you need to ask for self-exclusion at each casino separately. I would prepare an email template, though, to save some time.
Then browse the rules or ask the chat operator how to proceed. Most likely, you'll collect quite a list of email addresses you should use to self-exclude properly. Well, it depends on each casino.
If you play in a larger casino group, feel free to ask whether you can self-exclude from the whole group instantly. This is a bit tricky, though. I would personally stick with the usual approach, meaning to go one by one.
Some players tried to self-exclude from all casinos licensed under the Curacao license, honestly, I have no feedback on that. Keeping the current legal mess in mind, I am not sure whether this option even works, however.
When it comes to form, though, state the reason (especially if you need to stop due to gambling issues) and ask for the self-exclusion clearly.
